Monitoring Survival Following Lung Transplantation with Inclusion of Unaccounted Risks

چکیده
Lung transplant (LTx) survival and quality of life have been steadily improving, partially from changes in candidate selection for transplantation. There are well-known high-risk factors that lead to poor LTx outcomes, which may be referred as ‘accounted’ risks. However, there also other ‘unaccounted’ risks not included the existing model expected help programs resource allocation. We performed a retrospective cohort study all recipients over age 18 at UC San Diego Health between June 2019 March 2021. Patient’s were identified low, moderate, or high risk 4 discrete categories: Medical, Surgical, Psychosocial Nutritional. The entered into software tool calculated an 'unaccounted' adjusted observed ratio (O:E), using calculations mirror Scientific Registry Transplant Recipients (SRTR) risk-calculation methodology same six month intervals, termed Program-Specific Report (PSR) cycles. During period, 65 patients underwent lung Of those, 25 had least one risk. risk-adjusted O:E was 1.54 while 0.46. is projected increase 4.72 0.43 after two more PSR Having knowledge ‘at risk’ identifying with helps allocate resources productive fashion continue achieve level outcomes meet standards.

Hyperacute rejection following lung transplantation.
Although hyperacute rejection has been clinically and pathologically fully described in recipients of other solid organ transplants, to our knowledge, there have been no previous fully documented cases in recipients of lung transplants. This case of clinical hyperacute rejection is corroborated by a positive, donor-antigen-specific IgG-mediated lymphocytotoxic crossmatch (LXM), and demonstrated...
متن کاملBronchiolitis obliterans syndrome and survival following lung transplantation for patients with sarcoidosis.
BACKGROUND End-stage sarcoidosis is characterized by severe pulmonary fibrosis and is often poorly responsive to medical therapy. Lung transplantation, therefore, may be the only treatment option. Currently, there are few studies evaluating long-term outcomes following transplantation for these patients. Our aim was to evaluate post-transplant morbidity and survival of patients with sarcoid com...
متن کاملBronchiolitis obliterans following lung transplantation.
Bronchiolitis obliterans syndrome (BOS) is the main and late chronic complication after lung transplantation. It remains a major impediment to long-term outcome. Unfortunately, the survival rate of lung transplant recipients lags behind that of other organ transplant recipients, and BOS accounts for more than 30% of all mortality after the third year following lung transplantation. Most recent ...
متن کاملChylopericardium following orthotopic lung transplantation.
Chylopericardium is an uncommon condition, reported to occur following routine cardiac surgery, orthotopic heart transplantation, cardiac trauma, intrathoracic tumors, or infection. It has not, to date, been reported following uncomplicated orthotopic lung transplantation. This article describes chylopericardium following bilateral orthotopic lung transplantation.
متن کاملSurvival impact of lung transplantation for COPD.
Chronic obstructive pulmonary disease (COPD) is the primary indication for lung transplantation (LTx), but survival benefit is still under debate. We analysed the survival impact of LTx in COPD with a new approach, using the BODE (body mass index, airway obstruction, dyspnoea, exercise capacity) index. We retrospectively reviewed 54 consecutive lung transplants performed for COPD. The pre-trans...
